Cannons Creek Pharmacy Provides the following services:
• dispensing medicines prescribed by your doctor, midwife or dentist.
• professional advice about the medicines you have been prescribed.
• we maintain a medication profile, keeping a list of medicines you are
taking in our files.
• for your safety, we have an obligation to report any significant findings about your medicines to the prescriber who wrote the prescription.
This is done in a confidential manner.

We wish to provide professional pharmacy services to all our customers. If you wish to complain about our service, please ask to speak to the Pharmacist in Charge.
If, after speaking to this pharmacist, your complaint is not resolved, you are entitled to contact the Health and Disability Services by calling 0800 11 22 33.
